Viscosupplements are various preparations of hyaluronic acid (HA), a naturally occurring substance in synovial (joint) fluid.
Hyaluronic Acid Preparations
Contrary to popular belief, HA doesn’t simply serve to lubricate the joint, but also has anti-inflammatory properties that inhibit enzymes and markers responsible for joint degradation (MMPs) and pain (IL-1Beta and Tumor Necrosis Factor Alpha).
Due to the mechanism of action of hyaluronic acid, the onset of pain relief may take 2-6 weeks. Studies by Lee et al in 2011 and Koh et al in 2019 demonstrated the combination of intraarticular HA and toradol allows for rapid onset of pain relief without the cartilage softening associated with steroids.
A study by Turajane et al in 2017 demonstated efficacy for the use of HA for delaying and in some cases preventing the need for surgery in mild to moderate knee osteoarthritis.
Multiple studies by Kwon et al 2013, Di Giacomo et al 2015 and 2017, and McKee et al 2019 demonstated superiority of HA for shoulder osteoarthritis compared to physical therapy alone.
